Skilled Nursing Facility Physician with Atrium Health, part of Advocate Health, caring for patients at various skilled nursing facilities in the Greater Charlotte Region, NC. The role is Monday through Friday, 8-5pm, with limited/no after-hours call.
- Setting
- Various Atrium Health Skilled Nursing Facilities in the Greater Charlotte Region, within an integrated health system. EPIC EMR with DAX Copilot AI documentation tool.
- The work
- In-person and virtual visits with a Medical Assistant, care coordination with the SNF team, and implementing appropriate discharge plans.
- Schedule
- Monday through Friday, 8-5pm, with flexible scheduling.
- Call
- Limited/No after-hours call.
- Patients
- Complex skilled nursing facility patients, seen 2-3 times a week.
- What the pay includes
- Continuing Medical Education (CME) allowances and relocation assistance.
- Requirements
- BE/BC in Internal Medicine, Family Medicine, Geriatrics or PM&R. SNF experience is a plus but not required; new graduates welcome.
