- Setting

- The work
- Short-term management of complex patients during a 30 day follow-up period, with integrated visits alongside a pharmacist and social worker; care in office, virtually and/or with community paramedicine support.
- Schedule
- Part-time .6 FTE, 3 Days/Week, in-person at Lincoln and virtually for other Transition Clinic locations. Office hours Monday-Friday (8am-5pm); no nights/weekends.
- Call
- Shared call rotation, rotated a week at a time; phone only.
- Patients
- Adults 18 years old and up at high risk of complications and readmissions due to medical complexity or barriers to healthcare access.
- What the pay includes
- CME allowances, relocation assistance and paid medical liability insurance.
- Requirements
- Board-Certified Internal Medicine Physician; experience preferred.
