Full-time outpatient Internal Medicine physician role at Atrium Health Transition Clinic, a hospital follow-up clinic split between Lincolnton, NC and Shelby (Cleveland), NC. The clinic serves high-risk patients over a 30 day follow-up period with integrated provider, pharmacist and social worker visits.
- Setting
- Outpatient multidisciplinary ambulatory Internal Medicine hospital follow-up clinic, part of Atrium Health/Advocate Health. Cross-location team of physicians and APPs. EPIC EMR with DAX Copilot.
- The work
- Short-term management of complex patients after hospital discharge, with visits in office, virtually and/or with community paramedicine support; addressing barriers through patient engagement, access and education.
- Schedule
- 1.0 FTE, 100% outpatient, Monday-Friday 8am-5pm. Tue, Wed, every other Friday at Cleveland; Mon, Wed, every other Friday at Lincolnton. No nights/weekends.
- Call
- Shared coverage rotated a week at a time; phone only.
- Patients
- Adults 18 years old and up at high risk of complications and readmissions due to medical complexity or barriers to healthcare access.
- What the pay includes
- CME allowances and relocation assistance.
- Requirements
- Board-certified in Internal Medicine. Experience preferred.
