- Setting
- Outpatient adult medicine clinic in an integrated not-for-profit health system, with on-site primary care, cardiology, endocrinology, orthopedics, eye care, GI and imaging.
- The work
- Comprehensive geriatric assessments, chronic disease and preventive care, cognitive and dementia care, medication management, fall prevention and functional evaluations, and care coordination with specialists and APPs.
- Patients
- Older adults, including patients age 65 and older, with complex chronic conditions such as diabetes, hypertension, cardiovascular disease and dementia.
- What the pay includes
- CME reimbursement and relocation assistance.
- Requirements
- MD or DO; Texas license or eligibility; completed accredited Geriatric Medicine fellowship; board-certified or board-eligible in Internal Medicine and Geriatric Medicine.
