About this job

Setting: Post-acute partner facilities (skilled nursing, long-term care) in the Greater Sacramento area, clustered to limit travel. Dual-charting in Sutter Epic and PCC.
The work: Initial 72-hour admissions and complex assessments, working with an NP who handles routine follow-ups and discharges. Charting can be done remotely after rounding. Pathway to Medical Director responsibilities.
Schedule: 8am - 5pm, Monday–Friday.
Call: 1:2 shared call rotation between two physicians; telephone-only, with physical facility visits rare.
Patients: Medically complex, multi-morbid patients transitioning from hospital to recovery; historically ~8 to 12 patients/day, including admits and follow-ups.
What the pay includes: $290,072 - $322,302 annually, with 10% of the benchmark salary tied to VBC and quality metrics. Sign-on bonus, relocation assistance and CME allowance.
Requirements: MD or DO; board certification in Internal Medicine or Family Medicine (Geriatric Medicine preferred); active unrestricted California license; experience in post-acute, skilled nursing, long-term care or geriatric settings.
