- Setting
- Outpatient senior primary care clinic within Gould Medical Group, a 600+ clinician multi-specialty group, using an interdisciplinary team-based, value-based care model.
- The work
- Direct patient care, leading huddles and interdisciplinary team rounds, coaching clinicians, APC supervision, quality and utilization metrics, empaneling patients, community outreach and protocol development.
- Schedule
- .2 FTE leadership reporting to the CMO and .8 FTE clinical. M-F traditional clinic hours.
- Call
- Shared call with other Sutter Senior Clinic physicians and APCs, no more than one week in three weeks.
- Patients
- Older, vulnerable value-based Medicare patients with sub-acute and chronic conditions, including those with complex needs or from underserved communities.
- What the pay includes
- Starting base salary $346,500; $50K incentive potential based on clinic performance; prorated APC supervision pay of $12K per 1.0 FTE APC supervised; annual CME/business expense allowance; relocation assistance.
- Requirements
- MD or DO, board certified in Family Medicine or Internal Medicine; unrestricted state license; 2-5 years in primary care/geriatrics or geriatric fellowship. Geriatrics fellowship and leadership and value-based care experience preferred.
