- Direct patient care, leading interdisciplinary rounds and huddles, coaching clinicians, accountability for clinic financial and clinical outcomes, patient empanelment, utilization work with hospital and SNF leaders, APC supervision.
- Schedule
- M-F traditional clinic hours; 2 SHN sponsored FTE leadership and 8 FTE clinical as written in the posting.
- Call
- Shared call with other Sutter Senior Clinic physicians and APCs, no more than one week in three weeks.
- Patients
- Vulnerable, value-based Medicare seniors with sub-acute and chronic conditions, including those with complex needs or from underserved communities.
- What the pay includes
- Starting base salary $346,500; $50K incentive potential based on clinic performance; prorated APC supervision of $12K per 1.0 FTE APC supervised; relocation assistance; annual CME/business expense allowance.
- Requirements
- MD or DO, board certified in FM or IM; unrestricted state license; 2-5 years primary care/geriatric experience or geriatric fellowship. Geriatrics fellowship highly preferred; leadership and value-based care experience preferred.
