About this job

Setting: Community-based outpatient palliative care practice (PCSS) within a multi-specialty medical group of over 1,900 physicians in 40+ specialties, affiliated with Palo Alto Medical Foundation of Sutter Health.
The work: Formulate care plans, patient education and ongoing evaluations with the Sunnyvale palliative care interdisciplinary team, collaborating with on-site primary care clinicians and specialists.
Schedule: Full-time is 35 patient contact hours, 5 days per week, with regular hours.
Call: No night shifts or weekend call.
Patients: Patients and families facing serious illness, seen in clinic, home, assisted living, nursing facilities and via telemedicine.
What the pay includes: Guaranteed salary, shareholder eligibility after two years, relocation allowance and malpractice tail coverage.
Requirements: Fellowship in Hospice and Palliative Care Medicine, board-certified or board-eligible in Hospice and Palliative Care Medicine, and a CA medical license.
