- Assess participants and build care plans with the interdisciplinary team, manage nursing home participants, admit to hospital, coordinate specialist referrals, document and code, and help develop policies and train staff.
- Schedule
- Part time, 20 hours per week, day shift: two 8 hour days at Assisi House and one 4 hour day at Valley View. Days worked are flexible.
- Call
- The posting states no night, weekend, on call or holiday coverage is required, but also lists participating in the provider on-call rotation as required.
- Patients
- Frail and elderly PACE participants, including nursing home residents, dementia patients and deaf seniors.
