- Longitudinal PCP for your own panel: preventive, acute and chronic care by video, specialist referrals, population health work (AWVs, care gaps, transitions of care, risk capture) and post-discharge follow-up.
- Schedule
- Full-time weekdays, 40 hours per week / 36 clinical hours. Must work Mondays and Fridays; start times flexible between approximately 7:00 am and 12:30 pm ET.
- Patients
- Established patients within the Novant Health system, including risk-attributed and recently discharged patients. Experience handling 3+ cases per hour expected.
- Requirements
- Board-certified in Family or Internal Medicine; active NC or SC license (IMLC preferred); 3+ years post-residency with 2+ years in-person primary care; virtual visit and EPIC experience; clean malpractice history. Spanish a plus.
