- Setting

- The work
- Manage your own panel of established patients with preventive, acute and chronic care by video. Refer to specialists, work on population health (AWVs, care gaps, transitions of care, risk capture) and support patients after discharge.
- Schedule
- 40 hours per week / 36 clinical hours as three 12-hour shifts: Saturday, Sunday and one additional weekday, starting between approximately 7:00 am and 9:00 am ET.
- Patients
- Established adult primary care patients within the Novant Health System, including risk-attributed patients and recent discharges. Experience handling 3+ cases per hour expected.
