- Own a patient panel and provide longitudinal care: prevention, chronic disease management, medication safety and care transitions, working with an interdisciplinary team. Opportunity to help shape the service line.
- Schedule
- Monday-Friday outpatient practice with approximately 10 pre-scheduled slots per day (up to 12 visits), mixing 60-minute new/complex and 30-minute follow-up visits, plus dedicated weekly administrative time.
- Call
- Shared 1:3 phone call, expected to include evenings, weekends, and holidays.
- Patients
- Older adults with complex and changing medical, functional, and social needs, along with their families and caregivers.
